Roche launches BAP1 test to streamline mesothelioma diagnosis

Roche launches BAP1 test to streamline mesothelioma diagnosis

New Capabilities

A single immunohistochemistry marker with near-perfect specificity could simplify a historically difficult differential diagnosis

Overview

Updated 51 minutes ago

A biopsy from the lining of a lung can look like several diseases under a microscope: mesothelioma, non-small cell lung cancer, or a benign scar. Roche's new BAP1 test uses a single marker with near-perfect specificity to isolate the deadliest possibility, malignant mesothelioma, from the rest.

Mesothelioma is rare but lethal: most patients are diagnosed late, and median survival is 12 to 18 months. The VENTANA BAP1 assay detects loss of a tumor suppressor protein in cell nuclei, a finding virtually 100% specific for malignancy in mesothelial tissue. Preserving tissue for molecular testing is another win, since traditional antibody panels can exhaust small biopsies.

Why it matters

Most mesothelioma patients survive 12-18 months after diagnosis. A 100%-specific test could cut diagnostic delay and preserve tissue for molecular testing.

Key Indicators

100%
Specificity of BAP1 loss for malignancy
In a 212-mesothelioma validation study, nuclear BAP1 loss was never observed in benign mesothelial proliferations.
66%
Sensitivity of BAP1 loss in mesothelioma
139 of 212 mesotheliomas showed BAP1 loss; sensitivity rises to 69% for epithelioid and biphasic subtypes.
12-18 months
Median overall survival after diagnosis
Most mesothelioma patients are diagnosed late, per Roche's press materials.
>80%
Share of mesothelioma cases that are pleural
The majority of cases occur in the pleural cavity lining the lungs.

  1. Roche launches VENTANA BAP1 (SP527) globally

    Today Product Launch

Roche announced the automated IHC assay to help pathologists distinguish mesothelioma from non-small cell lung cancer and benign proliferations. FDA-registered as Class I IVD; IVDR-approved as Class C device.

  2. BAP1 IHC validated in 212-mesothelioma study

    Research

Modern Pathology study found BAP1 loss is 100% specific for malignancy in mesothelial proliferations, with 66% sensitivity. All six BAP1-negative reactive proliferations later developed into BAP1-negative mesothelioma.

Scenarios

1

Global guidelines name BAP1 as a standard marker for mesothelioma workup

Likely Resolves by End of 2028

Discussed by: Roche's press release frames BAP1 as addressing unmet clinical needs; the Modern Pathology study supports its adoption

The 2022 validation data showing 100% specificity could drive WHO, CAP, IASLC, NCCN, or ESMO to list BAP1 IHC as a recommended marker in mesothelioma differential diagnosis guidelines. This would cement BAP1 alongside mesothelial lineage markers like WT1 and calretinin as standard practice.

2

BAP1 adoption lags in community labs despite guideline support

Possible Resolves by End of 2028

Discussed by: Healthcare economics observers tracking diagnostic reimbursement trends

Automated IHC platforms, training, and reimbursement determine real-world adoption. If costs and lab capacity limit uptake outside academic centers, the test's impact on patient outcomes would be uneven despite strong clinical evidence.

3

Alternative markers or genomic sequencing displace BAP1 as first-line

Unlikely Resolves by End of 2029

Discussed by: Competitive diagnostics landscape; emerging literature on MTAP, Claudin-4, and NGS panels

Roche's own MTAP and Claudin-4 assays, or next-generation sequencing panels, could emerge as preferred first-line approaches. A head-to-head comparative study or guideline update recommending these over BAP1 would shift the market.

Historical Context

One moment from history that rhymes with this story — and how it unfolded.

2022

BAP1 IHC validation study (2022)

A Modern Pathology study of 212 mesotheliomas, 12 benign mesothelial tumors, and 42 reactive mesothelial proliferations found BAP1 protein loss in 66% of tumors. Loss was 100% specific for malignancy. All six BAP1-negative reactive proliferations later developed into BAP1-negative mesothelioma within 2 to 104 weeks.

Then

The study established BAP1 IHC as the most specific single marker for distinguishing malignant from benign mesothelial proliferations, with sensitivity highest in epithelioid and biphasic subtypes.

Now

The evidence became the clinical foundation for Roche's VENTANA BAP1 (SP527) commercial assay and for positioning BAP1 alongside lineage markers like WT1 and calretinin.

Why this matters now

This study is the validation data behind the test Roche launched in October 2026. Its 100% specificity figure is the core claim in the product's clinical rationale.
