Prenatal choline trial links supplement to higher child IQ and fewer social withdrawal problems

Prenatal choline trial links supplement to higher child IQ and fewer social withdrawal problems

New Capabilities

Largest randomized trial of prenatal choline finds a mean 7.78-point IQ gain and fewer social withdrawal problems at age 4

Yesterday: Largest prenatal choline RCT reports IQ gain

Overview

Updated 1 hour ago

Four-year-olds whose mothers took a daily choline supplement during pregnancy scored about eight points higher on IQ tests and showed far fewer social withdrawal problems than children of mothers given a placebo. The result comes from the largest randomized trial of prenatal choline supplementation ever run, reported this month in the American Journal of Obstetrics & Gynecology.

Choline activates the fetal α7-nicotinic acetylcholine receptor, encoded by the gene CHRNA7, which has been linked to schizophrenia, autism, and attention deficit hyperactivity disorder. The senior author says a safe, low-cost nutrient could shift routine prenatal care the way folic acid did. Whether that happens depends on independent replication, because a 2025 systematic review found the evidence too thin to recommend choline.

Why it matters

If choline's effect on child IQ and social development holds up in replication, a cheap supplement could reshape prenatal care just as folic acid did.

    Latest Research

A randomized trial of 206 pregnancies reports a 7.78-point mean IQ gain and fewer social withdrawal problems at age 4, with no significant adverse effects.

  2. Systematic review finds evidence thin

    Research

A systematic review of trials and observational studies concludes evidence for prenatal choline benefit is insufficient, calling for a high-quality trial with follow-up beyond early childhood.

  3. Pilot follow-up points to choline benefit

    Research

Freedman's group reports that children whose mothers took perinatal choline had fewer attention problems and less social withdrawal at 40 months.

Scenarios

1

Obstetric guidelines raise prenatal choline recommendations

Uncertain Resolves by Q2 2028

Discussed by: Study team and advocates of nutritional psychiatry

The trial's authors end by recommending 7,200 mg of phosphatidylcholine or 900 mg of choline daily, beginning early in gestation. If the American College of Obstetricians and Gynecologists (ACOG) or the World Health Organization adopts a higher recommended choline intake for pregnancy, the supplement would move from research finding to ordinary prenatal care. This is the path folic acid took in the 1990s.

2

Independent trial confirms the IQ effect

Uncertain Resolves by End of 2028

Discussed by: The 2025 systematic review authors, who called for a high-quality trial with follow-up beyond early childhood

The current trial carries built-in limits: parents rated children's behavior, about a third of participants dropped out by age 4, and the attention-problems primary outcome missed statistical significance. An independent, adequately powered trial with objective cognitive measures would settle whether the IQ and social-developmental gains are real.

3

Evidence stays inconclusive

Possible Resolves by End of 2028

Discussed by: Systematic review authors skeptical of the existing trials

A second pathway is inaction. If no confirmatory trial is funded, or an updated systematic review again finds the trials too small and inconsistent, the finding stays suggestive rather than policy-changing. This pattern has marked other prenatal nutrient candidates, which never moved past mixed studies.

Historical Context

2 moments from history that rhyme with this story — and how they unfolded.

1991-1998

Folic acid and neural tube defects (1991-1998)

The 1991 MRC Vitamin Study randomized 1,817 women who had previous neural tube defect pregnancies and showed folic acid cut recurrence by 72%. The U.S. began mandating folic acid fortification of enriched grains in 1998, and spina bifida rates fell sharply.

Then

Fortification became national policy within seven years of the trial.

Now

A routinely fortified nutrient now prevents thousands of birth defects a year in the U.S.

Why this matters now

It is the model for moving a prenatal nutrient from randomized trial to population-wide recommendation, the path choline's supporters envision.

October 2016

Freedman group's perinatal choline pilot (2016)

A pilot by the same University of Colorado team gave pregnant women phosphatidylcholine or placebo, then followed the children. At 40 months, parent ratings showed fewer attention problems and less social withdrawal in the supplement group, with the effect tied to CHRNA7 gene variants.

Then

The pilot was small, roughly two dozen children per arm, but pointed the same direction as the new trial.

Now

It established choline's biological mechanism in newborns through cerebral inhibition measurements and laid the groundwork for the larger 2026 trial.

Why this matters now

It is the direct predecessor to the new study, showing the effect appears across two consecutive trials from the same research program.
